If you would like to continue receiving care at ICHC — Sun Lakes Family Physicians, you have multiple options to choose from, including out-of-network rates or self-pay arrangements. Our staff can help you select the option that’s right for you.
In the event you wish to switch to a provider contracted with your insurance plan, please speak with our clinic staff or contact us on 480-505-2450 to request a copy of your medical records. Should you prefer, we can transfer your medical records directly to your new provider.
